NFPA Entry Forms
Deadline is  January 31, 2026.  Late entries will be accepted until February 3rd accompanied by a $25.00 late fee per category.
Entry forms must be completed in full.  Click on the button below to input your entry.  
There are different entry forms - please use the one for your discipline -  Classical Voice, Popular Voice, Classical Piano,  Popular/Contemporary Piano, Musical Theatre, Songs from the Screen and More or Classical Strings.
Payment should made by e-transfer to niagarafpa@gmail.com .  Entries are not complete until payment has been received.  Thank you for supporting the NFPA!
Changes/corrections to entries will be accepted until February 8, 2026 with a fee of $15.00/change.
